Lambeth Together is looking to improve the way it provides the NHS Health Check and would appreciate your feedback in helping to achieve this aim.

The NHS Health Check is a simple check of your heart health. It aims to reduce your risk of developing preventable problems like heart disease, a stroke, type 2 diabetes, kidney disease or dementia. The health check is available to anyone aged between 40-75 years old who has not already been diagnosed with heart disease, diabetes, kidney disease, or had a stroke.

This survey is open to anyone who meets this eligibility criteria for the NHS Health Check.  All information gathered is anonymous.
